What they do
Merck & Co. (Rahway, New Jersey; CEO Rob Davis; ~75,000 employees; known as MSD outside the US and Canada) is one of the world's largest pharmaceutical companies. Its heart is Keytruda, the immunotherapy that helps the immune system attack cancer: $31.7 billion in 2025 and $8.4 billion in Q2 2026 (~half of revenue), including Keytruda Qlex, the injectable version given in ~1 minute instead of a 30-minute infusion. Next come vaccines (Gardasil against HPV, $1.17 billion in the quarter; Capvaxive against pneumococcus), Winrevair (pulmonary hypertension, from the $11.5 billion Acceleron acquisition), Welireg (kidney cancer) and an animal health business of $1.8 billion a quarter (+8%).
Since 2025 it has bought its next generation: Verona Pharma (~$10 billion, closed October 7, 2025; Ohtuvayre for COPD, $204 million in the quarter), Cidara (~$9.2 billion; CD388, flu prevention in Phase III) and Terns ($6.8 billion; MK-4208 for chronic myeloid leukemia).
2026 brought key approvals: Lipfendra (enlicitide, July 16), the first and only once-daily PCSK9 pill to lower LDL cholesterol — by up to 60% —, new Keytruda combinations in kidney, bladder and triple-negative breast cancer; and on August 19 the personalized messenger-RNA therapy intismeran (with Moderna) met its Phase III goals in melanoma — the first of its kind to do so.
Why we like it
Three reasons.
- KEYTRUDA'S REPLACEMENT ALREADY SHOWS IN THE NUMBERS: in Q2 Winrevair sold $588 million (+75%), Welireg $271 million (+67%), Capvaxive $184 million (+42%) and Ohtuvayre $204 million; 2026 sales guidance rose to $66.3–67.3 billion (from $65.5–67.0 billion in February). On September 14, at the Morgan Stanley conference, management said its goal of more than $70 billion in revenue from recent launches and pipeline by the mid-2030s 'looks conservative' (oncology now >$25 billion) and describes Keytruda's patent expiry as 'a hill, not a cliff'.
- A PIPELINE THAT JUST DELIVERED: Lipfendra approved (~30 million US patients treated but not at their cholesterol goal), intismeran positive in Phase III melanoma (August 19; in Phase II it cut the risk of relapse or death by 49% at 5 years), sacituzumab tirumotecan met its Phase III goal in endometrial cancer and Keytruda Qlex is moving patients to the injectable version.
- CHEAP BEHIND THE ACCOUNTING EFFECT: the 114.8x price vs. profit reflects one-time acquisition charges — 2026 adjusted EPS guidance ($2.66–2.76) includes $6.05 per share of charges (Cidara $3.62, Terns $2.31, other $0.12); without them it would be ~$8.71–8.81, and the 2027 consensus is $9.55 → at $143.85 it trades at 15x. It pays $3.40 a year (2.35%), raised 4.9% in November 2025, 15 straight years of increases. Buy consensus from 28 analysts, average target $152.96 (+6.3%), with September raises: HSBC $172, BofA $166, Guggenheim $170, DBS $180.
Key Risk
#1: Keytruda's patent expires in 2028. It is ~half of revenue; the thesis needs Winrevair, Lipfendra, vaccines, acquisitions and new oncology drugs to fill the gap in time — the company promises 'a hill, not a cliff', but it is a promise. Others:
- generics already in 2026: Januvia/Janumet (also with a government-set price), Bridion and Dificid take away ~$2.5 billion this year;
- Gardasil in China: demand collapsed and Merck has halted shipments since 2025 — the vaccine is no longer a growth engine;
- acquisition execution: ~$26 billion on Verona, Cidara and Terns since 2025, with ~$14.7 billion booked as expense in 2026 — they have to work; gross margin fell 1.1 points in the quarter;
- US pricing policy: the December 2025 'most-favored-nation' deal (Januvia from $330 to $100 on TrumpRx) and reimbursement pressure keep shifting;
- no longer beaten down: up ~75% in a year, trading near its high ($156.92), and the average target implies only +6%; Lipfendra will have a slow start (full coverage around end of 2027).
